Sleeping in an assigned bed does give happy thoughts, being unable to makes a dwarf unhappy. However, you don't generally have to bother with assigning beds since dwarves will claim unassigned bedrooms for themselves automatically. Just a side-note: married dwarves and children share beds. I did great bedrooms for all the new citizens, 3x3 polished stones with beds, doors, and some of them even have cabinets and chests already. Even then, they didn't claim their spot!

Personally, all my normal dwarves have a 3x4 living space, and I have never had any sort of unhappiness issues. I can confirm this, except that I use a 3x3 design (bed, table, chair, cabinet and if I ever get around to it, also a chest), and that makes all dwarfs as happy as unicorns in a magical forest. You can add doctors to a hospital the same as you can add performers and tavern keepers to the tavern. Dormitory or bedrooms don't need to overlap.
