About a month ago I made a post that the Lost Izalith shortcut wouldn't open for me. I ended up not playing the game anymore for a while, but today I went back to that save file, played some more of the game, and it still won't open. So for the record, here's the timeline: I beat the game without joining the Chaos Servants at all. Once done, it's an easy warp to the Daughter of Chaos, hop on the elevator, short run and you're right in the heart of Lost Izalith.
