Some actions generate more noise than others, and therefore are more dangerous to perform. So every now and then I hear gunshots in the game. According to www.pzwiki.net , NPCs aren't in the game yet, but random gunshots will be played every now and then to mess with the player and move the zeds around. However, the page on NPCs is outdated, so NPCs may be in the game. So, just to confirm, are there NPCs in the game or no?

As far as I can deduce, they have not spotted you, but simply migrated elsewhere and stumbled on you purely by chance. Zombies sometimes migrate and they can easily cross from one side of the town to the opposite when they do.Also the 38 makes very little noise, if you're able to train somewhere first with a shotgun to level up shooting, the 38 is a reliable way to blast your way through 4-5 zombies fast without drawing a horde. But in current state its the same as your character caught random amok shouting out like mindless wreaking havok in the safehouse effectively attracting all zeds out from 4km area.
