I was having this same issue, passthrough mode fields greyed out and you can't change the settings. To fix, I had to temporarily turn DHCP back on under Home Network / Subnets & DHCP. Go back into IP Passthrough and the fields could be changed. You can select the mode DHCPS-fixed and choose your device/MAC address and hit save.
