I received my abyssal head from the catacomb abyssal demons. If you've killed them 16k times its like killing something 9 times with a 1/3rd drop rate of an item and not getting the item. It's pretty unlucky, but not anything extra-ordinary. That's a lot of time though man, I'm sorry :/
